Well actually, brain surgery is often done with only local anestesia (I know that is misspelled horribly, but am too tired to care right now) which means the patient is kept conscious. And I also just assumed Zaphod had programmed some kind of surgeon computer to do it... though one NOT built by the Sirius Cybernetics Corporation, as that would have surely killed him.
